Case Management

When you are hospitalized, a member of the Case Management Department reviews your stay and assesses you for any discharge planning needs. A Registered Nurse Case Manager may work with you, your primary nurse, attending physician, and other members of the multidisciplinary team, to assist in meeting your care needs during your stay and also in preparation for your discharge.

The Case Management Society of America (CMSA) defines case management as a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.
