Case Management


The Case Management Department reviews all patients who are admitted to the hospital for appropriateness of care and coordination of care between disciplines within the hospital. Meetings are conducted with the patient and family members to assess the needs of the patient upon discharge. With the collaboration of the patient, family, physicians, nurses and other appropriate clinical disciplines, a discharge plan is established to ensure the best possible outcome.
